    • Posterior malleolar fracture with more than 25% or more than 2 mm of deformity
    • Bimalleolar or trimalleolar fracture
    • Ankle instability
    • Open fracture or soft-tissue injury
  • Surgical options:
    • Open reduction and internal fixation (ORIF):
      • Fix the fracture site with screws, plates, or pins
    • External fixation device:
      • Used for patients with severe trauma or poor soft-tissue condition
    • Arthrodesis (joint fusion):

  • Postoperative care:
    • Restrict weight-bearing for 4 to 6 weeks after surgery, then gradually increase load as recovery progresses

Medication

    • If needed, use opioids short-term
  • Antibiotics:

Follow-up

    • Imaging 6 weeks after surgery to assess healing
    • Further follow-up of functional recovery at 3 and 6 months after surgery
  • If functional impairment is present, additional rehabilitation sessions are needed

Prognosis
